ABHC offers two options for completing New Patient Forms. The first, and more traditional option is to complete your paperwork in-office. We ask that you arrive 30 minutes ahead of your scheduled appointment time to complete this paperwork.
For those wishing to get a head start before arriving for their first appointment, all necessary paperwork can be completed online by following the "Forms" tab at the top of this page or by clicking here. Each link provided therein will open a HIPAA-compliant link to submit each form to us, safely and securely.
We offer two options to request prescription refills:
Patients may call the office at least 72 hours before you need a prescription refill.
Patients may login to their Patient Portal and send a message to our office with "prescription refill" as the subject line. Please be detailed about what prescription(s) you need refilled.
Please note: prescription refill requests will only be completed for patients who have been seen by their provider within the past 3 months and who have an upcoming scheduled appointment. Additionally, we DO NOT accept refill request from pharmacies via fax or phone.
If you fail to arrive for your scheduled appointment, whether in person or via TeleMed (or do not change or cancel at least 24 hours prior to appointment time), you will be charged a $100.00 fee. This fee must be paid prior to your next appointment.
All payments, deductibles and/or co-insurance are due at time of service.
In an effort to best serve our community, we accept most insurance carriers, to include: BCBS, AETNA, CIGNA, MEDICARE, and UNITED HEALTH CARE. Please contact the office if you need help confirming insurance eligibility. We can be reached by calling (352) 742-8300
If you are unable to make your appointment, please call 24 hours in advance from the time of your appointment to cancel. All missed appointments or appointments cancelled less than 24 hours from the time of your appointment will incur a $100.00 No show fee.
